This form is for those who are attempting register with EMW Lacrosse  but do NOT attend the Iroquois Central School District on a daily basis.

If a player attends the Iroquois Central School District on a daily basis, we can easily verify their EMW residency and grade level.  If your child does not attend the Iroquois Central School District on a daily basis, we need you to take one of the following actions:

1)  If you are home-schooled with the Iroquois Central School District, please provide proof of that with a report card photo indicating that you are a home-schooled student of the Iroquois Central School District.

2)  If your child attends another school district but you pay taxes to the towns of Elma, Marilla or Wales, please provide a photo of your tax records. No amount needs to be shown - simply the tax bill with your name and address.  Please just submit your TOWN tax records - not your Federal Tax Return.  Again, please feel free to black out all dollar amounts when submitting.  We only need to see your name and address as well as the date and town on the tax bill/receipt.

You do NOT need to do both of these items - just the one that fits your circumstances proving that you are eligible to reigster with EMW Lacrosse this session.
